Contact After Adoption Agreement

This form is an agreement that lets the court know that the adoptive parents and the child's biological family members have agreed on how they will stay in contact after the adoption is finished. It asks for details about the people involved and what kind of contact is allowed.
The court describes it as: Tells the court that the adoptive parent(s) and the biological family members have agreed to contact after the adoption is done. Once completed and signed by everyone who is part of this agreement, this form should be attached to the Adoption Request (ADOPT-200) or can be filed at the time of the Adoption Finalization hearing and attached to the Adoption Order (ADOPT-216). This form includes what type of ongoing contact is allowed.
