Request for Trial De Novo After Judicial Arbitration

This form is used to ask the court for a new trial by the judge after a decision was made by an arbitrator in a court arbitration program. It must be filed within 60 days of the arbitration award being filed with the clerk.
The court describes it as: Ask for a trial by the court after a decision was made in your case by an arbitrator in a court arbitration program.
