Mediation Disclosure Notification and Acknowledgement

This form is a notice and agreement that an attorney gives to a client before mediation. It explains that things said or written during mediation are usually kept private and cannot be used as evidence in later court cases.
The court describes it as: Attorneys for clients who are participating in mediation or communicating with a mediator about participating give their clients this information about the confidentiality of mediation. Clients sign to say that they understand this information.
