Application for Appointment of Guardian Ad Litem—Civil and Family Law

This form asks the court to officially appoint a Guardian Ad Litem. This person is appointed to help make decisions in a case when one of the people involved is a minor, cannot make decisions on their own, or already has a conservator.
The court describes it as: Asks the court to appoint an adult to have decision-making authority in a case in which a minor (person under 18) is a party. In most case types, the minor must also be represented by an attorney.
