Receiving Court Comment Form

This form is used by a court where a supervised person lives to give comments to the court that handled the original conviction. These comments explain whether moving the case to that location is appropriate.
The court describes it as: The court where a supervised person lives can use this form to provide any comments to the court where the conviction occurred on why a the transfer of a case is or is not appropriate.
