This form is a list used to record all known property and debts. It asks the person filling it out to list assets and debts, and it is an attachment to the Declaration of Disclosure form.
The court describes it as: List all your known property and debt. This can be used as an attachment to Declaration of Disclosure (form FL-140) or you may use Property Declaration (form FL-160) instead.
