Income Withholding for Support

This form is a notice that tells an employer that a court has ordered payments for support, such as child support or spousal support. It tells the employer exactly how much money to take out of the employee's paycheck and how often to send it.

The court describes it as: Tells an employer that the court made an order for you or the other person in the case to pay child support, medical support, spousal or domestic partner support (and any past-due support). The order tells the employer how much money to take out of the employee's pay check, how often to take out the money, and where to send it. This is also known as a wage assignment.
