Who can serve the documents using this form's instructions?
A person who is at least 18 years old must serve the documents. You cannot serve the documents if you are a party to the case. Great, if you are a private person serving for a party, you must check box “a.”
Do I need to fill out a form for every set of papers I serve?
Yes, you must complete a proof of service form for each package of documents you serve. For example, if you serve two people, you must complete two proofs of service.
What information must I include when I fill out the form?
You must print the name, address, and phone number of the person being served, and you must list the name of each document you delivered to that person.
