Notice of Activation of Military Service and Deployment and Request to Modify a Support Order

This form is used by a service member to tell the court that they have been activated for military service or deployed out of state, and to ask the court to change an existing child support order.
The court describes it as: Tell that court and the other parent that you are a military service member, you have been activated to duty, deployed out of state, and you want to change a child support order.

- Does the local child support agency help with this?
- The local child support agency can help prepare a request for modification, and if they are involved, they must attach a Notice of Deployment to the form.
