Consent of Proposed Guardian, Nomination of Guardian, and Consent to Appointment of Guardian and Waiver of Notice

This form lets people tell the court who they want appointed as a guardian for someone's person estate. It also lets people agree to be the guardian or agree that someone else should be appointed.

The court describes it as: Tell the court about a person you want to nominate to act as guardian, or tell the court that you have been nominated and that you agree to act as guardian, or that you agree with someone else being appointed to act as guardian.
