Electronic Service: Consent, Withdrawal of Consent, Address Change (Juvenile)

This form lets a person tell the court if they agree to get case papers and notices by email or electronically. It can also be used to change an existing electronic address or to say they no longer want electronic notices.

The court describes it as: Use this to tell the court and other parties in the case that you: Agree to receive notices and documents in your case electronically and give the court an electronic service address, like an e-mail; want to change your electronic service address; or do not want to receive notices and documents in your case electronically anymore.
