Physician's Request to Continue Medication—Attachment

This form is used by a doctor to ask the court for permission to keep giving a child a certain type of medicine. It is a shorter version of another form and can only be used by the same doctor who filled out the most recent related form.
The court describes it as: The doctor uses this to request a court order for a continuing prescription of psychotropic medication. It is a shorter version of form JV-220(A) and may be used only by the same doctor who filled out the most recent JV-220(A) if they are prescribing the same medication with the same maximum dosage.
