Notice of Intent to File Writ Petition and Request for Record to Review Order Designating or Denying Specific Placement of a Dependent Child After Termination of Parental Rights (California Rules of Court, Rule 8.454)

This form is used to tell the juvenile court that a party intends to ask a higher court (an appeals court) to review a decision about where a child should live after the court has ended parental rights. It also asks the court clerk to gather the necessary case records for that review.
The court describes it as: A party in a juvenile dependency proceeding, or the child’s caregiver, can use this to ask that an appeals court review the juvenile court’s decision to order or deny a specific placement for the child, after parental rights have been terminated. In most cases, you have only 7 days from the court’s placement decision to file this form.
