Motion & Order for Interrogatories (Short Form)

This form is a request to the court to force a person who has already lost a case to answer specific questions about their finances and assets. It asks the judge to order the person to answer these questions or pay the money owed.

- Who fills out this form?
- The judgment creditor (the person who won the judgment) requests this form, and the judge signs the order. The judgment debtor must fill out the answers and sign a verification. Great care must be taken to fill out the blanks.
- What happens if I don't answer the questions?
- The form warns that failing to answer truthfully and completely within 14 days can cause the court to issue a citation for contempt of court, which might lead to a fine or jail sentence.
