Who needs to fill out the first section?
The first section must be completed if the person asking for the search is an adult descendant of the adoptee, the adoptive parent, the adoptee's spouse, an adult stepchild, or an adopted adult sibling of the adoptee, and the adoptee is not deceased.
What does the second section cover?
The second section must be completed if a biological grandparent is asking for the search and the adoptee's biological parent is not deceased.
Does this form require a lawyer?
The form has spaces for an attorney or party without an attorney to list their information.
