Plea Questionnaire/Waiver of Rights (CHIPS and JIPS)

This form is used by a child, juvenile, parent, guardian, or custodian to formally enter an admission or no contest plea regarding a case involving a child in need of protection or services (CHIPS) or a juvenile in need of protection or services (JIPS). It requires the person filling it out to acknowledge and give up several rights.
