Transfer by Affidavit ($50,000 and Under)

This form is used to transfer property belonging to someone who has passed away if the total value of the property is $50,000 or less. It allows the transfer to go to an heir, a trustee of a trust set up by the person who passed away, or someone who was the guardian at the time of death.
The court describes it as: To transfer decedent's assets not exceeding $50,000 (gross) to an heir, trustee of trust created by decedent, or person who was guardian of the decedent at the time of the decedent's death for distribution.
