This form is used to ask the court to make an existing agreement about contact after an adoption either stick, change, or end. The form requires that all people who signed the original agreement have first tried to solve the problems using a mediator or other dispute resolution program.
The court describes it as: Tells the court that the contact after adoption is not happening or that it needs to be changed or ended. This form can only be used after everyone in the agreement has tried to work out the problems through the use of a mediator or other dispute resolution program.
