Abandonment of Appeal (Unlimited Civil Case)

This form is used to tell the trial court that you want to give up an appeal you filed. It asks the person filing to sign and state the date of the appeal being given up.
The court describes it as: Tell the trial court that you want to give up your appeal. Use this form only if the record has not yet been filed in the Court of Appeal, otherwise use form APP-007.
