Proof of Service (Court of Appeal)

This form is used to tell the court that you delivered legal papers in a Court of Appeal case to another person. It asks you to list what papers were delivered and how they were delivered, either by mail or in person.
The court describes it as: Tells the court that you had legal papers in a case in the Court of Appeal delivered to the other party either by mail or in person. Lists the papers that were served and tells when they were served. For in person service, also tells where the papers were served and who served them.
