Proof of Service—Disability Access Litigation

This form is used in a civil case about whether a building meets rules for disabled access. It lets the court know that certain legal papers were delivered to the other party.
The court describes it as: In a case about whether a building meets requirements for access of disabled persons, tells the court that you had legal papers, other than a summons, delivered to (served on) the other party. Lists the papers that were served and tells who they were served on, where, when, and how they were served, and who served them.
