Property Declaration

This form is used to list all of a person's property and debts, and to suggest how those items should be divided. It can be used as part of a larger disclosure filing or when filing with the court.
The court describes it as: List your property and debts and how you would like to divide them. This can be used as an attachment to Declaration of Disclosure (form FL-140) or you may use Schedule of Assets and Debts (form FL-142) instead.
